This program funds Tribal and Urban Indian communities to strengthen dementia care across the continuum, from early recognition to caregiver support. It offers two options: coordinating integrated dementia care priorities including culturally tailored services and networks, or delivering comprehensive caregiver support training. Recipients will use standardized measures for evaluation and continuous quality improvement to enhance outcomes for AI/AN people and caregivers.
Eligible applicants include federally recognized Native American tribal governments, tribal organizations, nonprofits with 501(c)(3) status (excluding higher education institutions), and Urban Indian Organizations. Letters of support or Tribal Resolutions are required from Tribes served.
