This funding supports programs that enhance workforce capacity by integrating dementia-capable Community Health Workers within the National Aging Network. Funded activities include providing home and community-based services for people living with dementia and their caregivers, strengthening relationships with rural and underserved communities, and addressing barriers to dementia care.
Eligible applicants include entities designated to plan and coordinate Older Americans Act programs at the local level, such as Area Agencies on Aging, Federally recognized Native American Tribal governments, and certain States functioning as AAAs.
