This funding opportunity provides resources to HRSA-funded health centers to expand nutrition services. The goal is to increase patient access to nutrition interventions that help prevent, manage, and treat diseases. Funds are intended to increase the number of patients or visits receiving nutrition services at eligible health centers.
Eligible applicants are Health Center Program award recipients with an active H80 award under section 330(e) of the Public Health Service Act. Individuals are not eligible to apply.
