Support projects that implement individual and system-level interventions to reduce chronic disease rates among children in four U.S. communities. Funded activities include demonstrating lifestyle and community changes that can be maintained over time to lessen chronic disease burdens and generating findings to guide replication and future health funding strategies.
Eligible applicants include a wide range of entities such as private and public higher education institutions, government bodies, nonprofits, small businesses, tribal organizations, and public housing authorities. Eligibility is broad, covering various organizational types.
