This program funds academic medical centers to integrate innovative telehealth technologies across chronic care services, including prevention, management, and treatment of acute episodes. Funded centers will disseminate research findings and serve as national models for telehealth integration in chronic care.
Eligible applicants include public and state-controlled institutions of higher education, private institutions of higher education, and academic medical centers.
