This funding opportunity supports an organization to serve as the Children’s Resource Center for southern Wisconsin, aiding families and professionals supporting children and youth with special health care needs. Funded activities include navigating complex medical diagnoses, accessing educational and community resources, specialized provider networks, health insurance options, peer networks, and advocacy opportunities across multiple southern Wisconsin counties.
Eligible organizations must be located or have an office in Wisconsin and provide services to Wisconsin residents. They must have or obtain a Universal Entity ID and not be debarred or suspended. Non-discrimination policies are required.
