Eligible activities include annual fundraisers, health fairs, and community health projects held in Santa Clara and Northern San Benito counties. Grants up to $5,000 support initiatives directly benefiting local residents. Applicants must be tax-exempt nonprofits and submit requests at least 60 days before project start.
Eligible applicants are nonprofit organizations tax exempt at Federal and State levels, serving Santa Clara and Northern San Benito counties. Organizations may receive one grant per fiscal year. Active Health Partnership or Health Equity Fund grantees are ineligible.
