This program funds nonprofit and eligible for-profit institutions to provide USDA-standard healthy meals and snacks at Adult Day Service Centers under the Community Care Program in Illinois. It reimburses up to two meals and one snack daily for enrolled adults age 60 or older or functionally impaired, and covers administrative and audit expenses. Institutions must file monthly reports to claim reimbursement and operate nonprofit food services accessible to all eligible participants.
Eligible applicants include nonprofit and for-profit organizations operating nonresidential adult day care services in Illinois. For-profit centers must have at least 25% of adults enrolled receiving Title XIX or XX benefits.
