This pilot program funds affordable housing combined with bridge rental subsidies and recovery support or treatment for adults with serious mental illness or substance use disorder at high risk of institutionalization or overdose. Grantees provide housing location, subsidy administration, recovery engagement, and coordinate services to prevent homelessness and adverse outcomes. The program prioritizes equity and inclusivity and requires monthly engagement and assistance with permanent rental assistance applications.
Eligible individuals are adults or those aging out of guardianship enrolled in Medicaid with serious mental illness or substance use disorder, meeting high-risk criteria related to hospitalization, incarceration, or overdose. Applicants must be at least 18 or aging out of guardianship.
