This program funds support services for individuals with serious mental illness residing in permanent supportive or transitional housing. It supports services primarily eligible for Medicaid reimbursement but excludes Medicaid-reimbursable costs. The goal is to help individuals maintain housing, function independently, and achieve recovery and self-sufficiency. Applicants must be certified mental health centers and meet strict registration and prequalification requirements.
Eligible applicants include nonprofit and government organizations that meet registration, prequalification, and mandatory requirements. Applicants must be certified Comprehensive Community Mental Health Centers and comply with state and federal registration and exclusion criteria.
